<option>-ban in the context menu is broken

when selecting "ban user" from the context menu for a chat member while pressing <option> there seem to be two bugs in colloquy:

1. the hostmask in the dlog is shown as <nickname>!<hostname> while it should be <nickname>!<username>@<hostname>

2. when banning something like nightstalker!nightstalker.users.quakenet.org it results in a ban that looks like nightstalker!*kenet.org@*

moving this ban dlog mockup that i once made from #422, not sure if its useful though...

Fixed in [3287]. I doubt we add a fancy dialog for this, Colloquy does not sugar coat pro features.
